For new patients
If you live within the practice
area - see below - then you may register as a
patient of this NHS practice. You will need to complete a GMS1 registration
form which can be obtained from Reception. The practice doctors have personal
lists, and it is normally expected that patients will consult with the doctor
with whom they are registered (although it is recognised that this is not always
possible).
At any one time one of the doctors will be nominated as the doctor with whom new
patients are registered.
You
may, if you wish, request to be registered with a specific doctor. However,
this may not always be possible - as there are occasions when some doctors'
lists are full and they are registering no new patients.
We welcome you as a patient of The Marlborough Medical Practice. May we ask you, please, to read carefully the notes below which include important information about the practice and the way it operates.
1 We ask all new
patients to make a 20-minute appointment with the Practice Nurse
as soon as possible after registration, for a "New Patient
Medical". The New
Patient Medical is a good opportunity for a brief health check -
including, most
importantly, blood pressure and urine testing - especially for those
healthy people
who rarely have a need to visit their GP. Please bring a urine sample
when you
attend for this Medical.
2 If you are currently under medication we strongly recommend you make an appointment with your doctor, so that your medication can be reviewed and repeat prescriptions set up if necessary.
3 Meningitis C vaccination. If you were born after 7 January 1977 you are strongly encouraged to have a Meningitis C vaccination, if you have not already had one (or a combined Meningitis A & C vaccination for travel purposes within the past three years). This vaccination can be given at the surgery without charge – please ask the Receptionist to make an appointment for you with a Practice Nurse.
4 Repeat prescriptions. Please note that we do not take telephone requests for repeat prescriptions (they should be delivered, posted or faxed), and that we require 2 working days’ notice. Alternatively requests may be made from the repeat prescription page of this website.
5 We regret that (apart from two disabled parking spaces) there are no spaces in the surgery car park for patient parking – the spaces are needed for staff and doctors. We ask, therefore, that you park in the public car park, or in George Lane.
6 Please let us know if you change your address or telephone number. It is also helpful for us to have your mobile telephone number.
7 If you move to an address outside the practice area we will normally ask you to register with another practice more local to your new address. The practice’s doctors will not normally make visits to patients outside the practice area. The practice area includes the following villages: Avebury Trusloe, Avebury, Axford, Beckhampton, Berwick Bassett, East Kennett, Fyfield, Oare, Ogborne Maizey, Ogborne St Andrew, Ogbourne St George, Rockley, Mildenhall, West Overton, Winterbourne Monkton, Wootton Rivers.

Fees for non-NHS services. The practice charges fees for non-NHS
services, e.g. private sick notes, some vaccinations, some medical reports and
examinations, and letters to third parties (for example a letter to a school to
explain a child’s illness). We will tell you of any fee before you receive
the service. In some cases we may ask for a fee to be paid in advance (for
example some expensive vaccinations).
If you do not attend an appointment for a medical examination for which a fee
would have been charged, you will be charged a cancellation fee if you did not
give sufficient notice for the appointment to be given to another patient. If
you request a report or a letter, but subsequently do not collect it, you will
still be charged the full amount.
9 Complaints and Suggestions. If you wish to raise any point about the service offered by the practice please contact our Practice Manager, Michael Reynolds. You may telephone, write, or make an appointment. We welcome comments and suggestions.

We ask that patients do not use mobile phones anywhere in the surgery -
in order to avoid disturbing other patients or interrupting consultations.
