The Surgery will be closed between
6.30pm on Friday 23 December 2011 until 8.00am on Wednesday 28 December 2011. Telephone lines will open at 9.00am
and
6.30pm on Friday 30 December 2011until 8.00am on Tuesday 3 January 2012. Telephone lines will open at 9.00am
When the surgery is closed contact Wiltshire Medical Services on 0300 111 5717 for urgent medical advice and treatment
Weekdays:
8.00 am to 1 pm and 2 pm to 6.30 pm
The telephone lines (01672-512187) are open from 9 am to 1 pm and from 2 pm
to 6.30 pm on weekdays only.
The surgery is closed on Saturday, Sundays and bank holidays.
Dispensary opening hours
Weekdays: 8.30 am to 1 pm and 2 pm to 6.30 pm
The telephone line (01672-514878) is open from 9 am to 1 pm and from 2 pm to
6.30 pm on weekdays only. Please note that this line is for Dispensary
enquiries - we do not accept telephoned requests for repeat prescriptions.
The Dispensary is closed on Saturday, Sundays and bank holidays.

Disabled access
We aim to ensure staff aware aware of the needs of our disabled
patients. All areas of our practice are accessible by wheelchair and
disabled toilets are available. Two dedicated disabled parking spaces are
provided adjacent to the main reception.

Car parking
We regret that there is no
parking for patients at the surgery; apart from two spaces for use by disabled
patients.
Please use the public car park or on-street parking in George Lane.

Zero tolerance policy
The practice operates a Zero Tolerance Policy for abusive and
aggressive behaviour in the surgery. Any patient reported as having been
abusive or displaying aggressive behaviour to any staff member will be removed
from the surgery list.

Change of address
It is important that you let the practice know if you change your
address or telephone number. If you move to an address outside the
practice area you will normally be asked to register with another practice.
Occasionally, exceptions to the request to re-register may be made on clinical
grounds.
Change of doctor
You may request to be registered with a different doctor - you
can make such a request to change verbally to one of our Receptionists. or in
writing to the Practice Manager. If there is a specific reason for
your request it is helpful if it can be mentioned.
The new doctor's agreement will be sought, and, subject to his or her agreement,
your registration will be transferred and you will be informed.
Temporary
residents
If you are not registered with the practice, but are living
temporarily within the practice area , you are
entitled to receive treatment from this practice and you may make appointments
as described above.

Confidentiality
We are required by the Department of Health to inform all
patients that access to information about them may be given, under strictly
controlled conditions, to other staff within the NHS.
